3. Appointments & Cancellations

• Schedule by phone at (848) 260-0353 or online at xcellcareclinic.com
• Please give at least 24 hours notice to cancel or reschedule
• Repeated no-shows may result in a no-show fee
• Dr. Khan provides Adult Primary Care only (patients must be 18+)

4. Payment Policy

• Co-pays and balances are due at time of service
• We accept cash, checks, and major credit/debit cards
• A 3.5% processing surcharge applies to all credit card payments

5. Insurance

We accept Medicare, Medicaid, and most major private insurance plans. Patients are responsible for verifying their coverage, co-pays, and deductibles prior to visits.
